Pharmacist / Rotating Shift, Auckland

Responsible for preparing, compounding, and dispensing medications to patients according to the laws and regulations governing pharmacists and the practice of pharmacy by the appropriate state and federal agencies. Responsible for the optimization of drug therapy by identification, resolution, and prevention of drug-related problems and improving therapeutic outcomes.Work relatively independently, under general supervision, and within the limits of established pharmacy/ambulatory care practice site's policies and procedures, as a member of the patient care team. Oversees the activities of support personnel, operating within the limits of pharmacy/practice site's policies and procedures, and the regulations of the Board of Pharmacy.Principal Duties and Responsibilities Leadership Provide direction and guidance to pharmacist and support staff, including technicians, in following policies and procedures, taking accountability, and promoting professional behavior. Provide feedback to pharmacy managers, medical directors, and other healthcare professionals for performance appraisals when appropriate.Staffing and Competency Coordinate the delivery of pharmacy services efficiently and effectively. Prepare, compound, and dispense medications according to regulatory laws and guidelines. Interpret, clarify, and evaluate medication orders for completeness and appropriateness. Supervise and verify support personnel work for accuracy.Provide drug information and consultation to healthcare professionals regarding drug therapy, interactions, side effects, and discrepancies. Identify, document, and resolve drug therapy problems. Manage patients under P&T approved CDTM protocols when appropriate. Evaluate ambulatory prescriptions and utilize the ambulatory care system for prescribing when appropriate. Perform physical exams and order and interpret labs when necessary. Coordinate outpatient medication obtainment, including insurance issues. Participate in pharmacokinetic dosing and monitoring, recommending adjustments as needed.Work in sterile products areas, ensuring accuracy and sterility of compounded products. Formulate and compound special dosage forms. Maintain records for controlled substances and investigational drugs. Participate in rounds and collaborate with healthcare providers. Provide discharge medication counseling, medication reconciliation, and develop staff guidance tools.Support research-related patient care and drug therapy protocols. Train new personnel and perform other assigned responsibilities timely and completely. Quality and Safety Follow departmental policies and procedures on assigned shifts. Ensure compliance with all relevant laws, regulations, and policies.Document and report clinical interventions, medication errors, near misses, and adverse events. Technology Utilize and troubleshoot area-specific technology, including robotics, automation, and information systems. Education Participate in staff meetings, rounds, development activities, and continued education.Engage in quality improvement committees, presentations, and professional memberships.
