ADL Cromwell• Listed: 12 Aug 2024 A client-focused role providing a gradual and supported transition for young people from Oranga Tamariki care, to help them get a good start to their adult lives. About the role Assist young people in developing skills and knowledge over time and support them to develop and learn from their experiences with a focus on building and strengthening a network of support.Maintain regular proactive contact with young people, as set out in their transition plan. Provide practical and empathetic support to assist young people’s development and increasing responsibility. In consultation with the Service Leader gain support for the young person to understand their past and support them to access opportunities to heal from past trauma.Provide reassurance for young people concerning their future and positive reinforcement to build confidence in their own abilities. Working alongside ADL's skilled clinical team to improve wellbeing of young people. Directly support young people and their whānau with mild needs. Work alongside clinicians with young people and their whānau with moderate needs.Help to navigate young people and their whānau to a wide range of pro social and wellbeing supports. Provide a service that reflects the principles of the Treaty of Waitangi. Respect the diverse ethnic and cultural heritage of New Zealand and utilise culturally appropriate approaches.
