Pharmacist Prescriber, Auckland

Tend is looking for Pharmacist Prescribers to work with us from anywhere in the country! The Pharmacist Prescriber plays an integral role as part of the wider care team model at Tend, working at the top of scope to ensure effective and efficient delivery of pharmacist services to patients (including prescribing) and providing support to GPs, nurses, and the wider Tend team.You will also provide care directly to people and their whānau, and support the care team to provide accessible, equitable, and optimal use and management of medication treatments. Responsibilities include: Medicine management and optimisation Medication reconciliation Advice, education, and supportManagement of long-term conditions Quality, Training, and Risk management Providing equitable and culturally safe care Our ideal Pharmacist Prescriber will have: A minimum of 5 years post-registration experience Familiarity with the GP Primary Care sector, and ideally, experience working in this areaExperience leading and advising other health professionals or care providers Experience in clinical quality, audits, and best practices Strong relationship management skills and experience working across professional groups If this sounds like you and you'd like to chat before applying, call William at 021 135 0204.
