About therole Wāhi Tautoko is a 10-beddedservice located in Building 8. We aim to support consumers that requireextended assessment or treatment following an admission to the acute inpatientservice prior to discharge.This may include: tāngata whaiorawho are presenting to services for the first time that may require extendedassessment and support around management of their symptoms. tāngata whaiorathat have an identified discharge pathway on admission - i.e. home or supportedaccommodation - but may require some additional assessment and upskilling tosafely and successfully transition into the community.tāngata whaiorawho may have limited social supports and require support to increase theirengagement in their local communities. The goals of the serviceinclude: Provision of asafe and supportive environment to provide extended assessment, treatment andrehabilitation following an admission to theadult acute serviceTeaching skillsto help support illness management and to support consumers to live the lifethey want to live Supportdevelopment of structure and meaningful routines Support safedischarge into a community setting About You We will be looking for an experiencedRegistered Nurse, with mental health scope, who are passionateabout providing consumer centred care.Part time opportunity available, 0.47FTE Rotating roster of 4 days off, 2 days on Work autonomously and as part of a cohesivemulti-disciplinary team Skills and experience In order to be successfulin this role, the following is required: Registered with the Nursing Council of New Zealandwith a current APC and mental health scopePrevious experience working within any area of mentalhealth Self-motivated, flexible and solutions-focused Knowledge of and a commitment to Te Tiriti O Waitangi Working at Health New Zealand Health New Zealand | Te Whatu Ora is dedicated to ensuring excellent healthcare for the people of New Zealand.
