About the Role Our client is seeking a Health Improvement Practitioner to join their general practice team. You will deliver brief, consultation-based services focused on evidence-based behavioural interventions for both mental and physical health conditions. This role is central to supporting an integrated 'Model of Care,' promoting seamless collaboration between general practice, community NGOs, and specialist services.
