It is part of the Rapid Rehousing service, a community housing initiative aimed at collaborating with other services to support whānau experiencing homelessness to secure permanent housing. The role is suitable for a registered health professional with experience in mental health, addiction, or social services, who is registered in their practice.Key responsibilities include: Reviewing and coordinating referrals Identifying and addressing barriers to housing access Assessing and supporting ongoing needs to sustain and improve living situations Collaborating with community providers and navigating the social housing sector Building relationships with agencies like the Ministry of Social Development and Kāinga OraSupporting whānau to maintain tenancies and achieve housing stability Candidate Profile | Mōu We seek registered health professionals with experience in mental health, social sectors, and housing.
