About The Role An amazing organisation seeking a Health Improvement Practitioner to join a general practice setting, providing brief, consultation-based interventions to support both mental and physical health. This role focuses on delivering evidence-based behavioural support to individuals, whānau, and groups across all age ranges.Hours Full time - Monday to Friday 8:30 am to 5:00 pm Responsibilities Include Delivering brief, evidence-informed interventions for a range of health and wellbeing needs Supporting general practice teams with mental and behavioural health expertise Working collaboratively across services to ensure continuity of careEngaging with diverse communities in a culturally responsive and mana-enhancing way Why This Position? Enjoy M-F standard business hours Requirements To be considered for this role applicants must have: FULL New Zealand registration and an APC which meets NZHPCA requirements (i.e. social worker, occupational therapist, psychotherapist, psychologist or registered nurse).NZ residency or a valid Work Visa Current Full NZ Drivers Licence Preferred applicants will be required to undergo pre-employment checks Skills And Experience Key to success in this role is an empathetic, compassionate approach, along with a strong understanding of integrated care. The practitioner will work within a well-established model that allows clients to move seamlessly between general practice, community services, NGOs, and specialist mental health or addiction providers when needed.Successful candidates will bring their experience in a New Zealand clinical mental health setting and demonstrate: Knowledge and experience in brief interventions, ideally with knowledge of FACT or ACT modalities Confidence in Te Reo meōna tikanga, and/or have experience working in Māori health settingsA can do attitude with a quality focus and a commitment to delivering best practice patient care Ability to work well within a multidisciplinary team Excellent relationship and communication skills Reference: 210113 Looking to make a real difference in your community, with support of an amazing organisation?
