Close proximity to Grand Rapids area About the Department The Corewell Health West Ambulatory Care Management Department comprises Social Workers, RN Care Managers, Community Health Workers, and Medical Assistants. Our team of Social Workers and RN Care Managers is embedded across Primary Care offices system-wide. We focus on identifying and engaging patients with behavioral health concerns or chronic medical conditions, helping them develop healthy behaviors and coping skills to improve health outcomes. Our Ambulatory Care Managers are integral to the care team, collaborating with primary care providers to develop appropriate treatment plans for patients with chronic diseases or behavioral health issues, promoting self-management and symptom improvement. The team also supports transitions of care for hospitalized patients to prevent readmissions and reduce costs. Additionally, our remote Community Health Workers address barriers related to Social Determinants of Health (SDoH), ensuring comprehensive patient support. The department has demonstrated consistent success through effective patient interventions.About Outpatient Clinics Corewell Health operates over 300 outpatient clinics across Michigan, offering diverse care options. With more than 40 primary care and specialty practices, staff have opportunities to explore various interests or specialize further. Scope of Work The role involves planning and prioritizing care for individuals and populations, focusing on strategies to promote optimal health. Responsibilities include providing peer support, role modeling, and developing life skills and wellness during home visits. The role requires managing a caseload of patients with varying complexity, aiming to improve outcomes through education and preventive care within a managed care environment.Assess internal referrals; provide short-term therapy or care management; monitor patient progress. Develop care plans; recommend strategies to PCPs and specialists; ensure continuity of care. Participate in Systematic Case Review; consult with psychiatrists; assist in psychotropic medication management.Respond to urgent mental health concerns, including suicide risk assessments; facilitate appropriate care access. Engage in professional development via peer reviews and continuing education.
